Peer Specialist

Stevens Center
18 hrs/week

Requirements:
High School diploma or GED. Must have a valid Pennsylvania Drivers License.  Must have a minimum of 12 months paid or volunteer experience within a 3 year period.   

Responsibilities:

  • Participate in recovery oriented service planning and completion of specified objectives with ICM or RC consumers
  • Attend and participate in staff meetings, supervision meetings and goal planning/review sessions as needed
  • Complete progress documentation on service planning and objective(s)
  • Maintain direct contact with recovering individuals receiving ICM or RC services
  • Develop knowledge of community resources and natural supports
  • Serve as a role model for individual receiving targeted case management services
  • Form relationships with Targeted Case Management consumers participating in Peer Specialist supported service planning
  • Represent Targeted Case Management as a member of NHS/The Stevens Center Continuous Quality Improvement Committee
  • Participate in continuing education activities to enhance skills relevant to job
  • Maintain consumer confidentiality in accordance with NHS/The Stevens Center policies
  • Other duties as assigned

Peer Support Specialist

Stevens Center
20 hrs/wk

Requirements: High School diploma or GED. Ability to provide support and encouragement to individuals based on staff person’s own experience of and recovery through mental illness.  Attend certified peer support training certification course.

Responsibilities:

  • Demonstrate a Rehabilitation/Recovery philosophy towards service delivery by providing strength-based approaches to service
  • Provide general support in basic day-to-day activities such as grocery shopping, budgeting, activities of daily living, cooking cleaning and laundry
  • Accompany individuals and support them on recreational activities to encourage and teach socialization skills
  • Assist with transportation needs if necessary
  • Role model appropriate behavior in the community
  • Role model coping techniques and self-help strategies
  • Assist with medication monitoring and medication education
  • Assist individuals with doctor appointments
  • Mentoring individuals through conversation and active listening skills.
  • Provide outreach to individuals who have missed appointments or have been hospitalized
  • Assist individuals in crisis management/de-escalation situations
  • Facilitate Recovery groups and assist individuals with developing Wellness Recovery Action Plans
  • Provide support and educational groups for family members
  • Attend and participate in all meetings, supervisions, and trainings in a professional manner
  • Maintain professionalism as per corporate policy during all aspects of service delivery
  • Complete all paperwork in an accurate and timely manner
  • Complete all other tasks as assigned
  • Fulfill on call responsibilities
